Medical Track Intake

From conversation
to your front door.

A licensed physician will review what you share here and decide whether the Medical Track is right for you. Takes about 6 minutes. Your information stays private.

  • This is a request, not a guarantee. The physician makes the medical decision — they may approve, modify, or decline.
  • Wellness coaching is free. If you and your coach decide the Medical Track is the right fit, the medical group collects a separate physician review fee at that point — refunded if you're not approved.
  • HIPAA-aligned handling. Data is encrypted in transit and stored securely. Used only for your medical review.
  • You can pause anytime. Use a private device. We'll never share your information.

What this program is
A medically supervised wellness program

OS reboot is a medically supervised wellness program. If you proceed into the Medical Track, a licensed physician in the affiliated medical practice reviews your full medical intake and decides whether a prescription is appropriate based on your medical history.

The medications used in the Medical Track may include:

  • Appetite & fullnessSemaglutide, Tirzepatide (GLP-1 and GIP/GLP-1 receptor agonists)
  • Metabolic supportMOTS-C, AOD-9604, Lipo-B / MIC+B12
  • Recovery & repairBPC-157, TB-500 (soft tissue, training consistency)
  • Cellular & antioxidantGlutathione, NAD+ (mitochondrial support, vitality)
  • Growth-hormoneSermorelin (sleep, recovery, body composition)
  • Cognitive supportSemax, Selank
  • Skin & cosmeticGHK-Cu

Whether any of these is appropriate for you, and at what dose, is determined solely by the prescribing physician. Coaches and partners do not make clinical decisions.

Regulatory framework

FDA approval applies to specific, mass-produced products — such as a Wegovy 2.4 mg injection pen. It does not apply to individualized compounded versions, even when the active ingredient is the same. Under Section 503A of federal law, patient-specific compounded medications are legally exempt from the standard FDA approval pathway.

No compounded medication in the United States is labeled FDA-approved, regardless of its active ingredient. This distinction is regulatory — not a statement about safety.

Safety in this program comes from (1) a state-licensed 503A compounding pharmacy meeting sterile-compounding standards including facility controls, sterility testing, ingredient quality verification, and beyond-use dating; and (2) the prescribing physician who reviews your history, evaluates your risks, orders and monitors labs, and adjusts therapy when needed.

Risks & your right to choose

All medications carry risks. Common side effects of GLP-1 medications may include nausea, vomiting, diarrhea, constipation, fatigue, and injection site reactions. Rare but serious risks may include pancreatitis, gallbladder issues, and thyroid concerns. The prescribing physician reviews your specific risk factors during this intake.

You may decline telehealth at any time and seek in-person care instead. Beginning this intake does not commit you to a prescription. Results vary by individual and are strongly influenced by lifestyle factors including nutrition, sleep, hydration, and movement. No outcomes or weight-loss guarantees are made or implied. Telehealth is not for emergencies — for emergencies, call 911.
